Significant Incidental Findings

As the National Lung Cancer Screening Programme in the Czech Republic continues to evolve, the focus is expanding beyond tumor detection to the management of significant incidental findings. Specifically, the Czech screening framework is currently developing recommended clinical guidelines for Interstitial Lung Abnormalities (ILA) to ensure standardized care for patients identified during Low-Dose CT (LDCT) scans.

Intersticial Lung Abnormalities

Intersticial Lung Abnormalities are CT findings that may correspond to Interstitial Lung Disease (ILD) in individuals who are currently asymptomatic. In lung cancer screening cohorts globally, the prevalence of ILA is estimated at 3–10 %, making these findings even more frequent than lung cancer itself.

Recent Data

Recent data from the National Screening Centre (ÚZIS ČR) provides a clear foundation for these new guidelines. Between January 2023 and August 2025, out of 16,995 patients who underwent baseline LDCT examinations, 72 % (12,226 patients) had at least one incidental finding recorded.

Specifically regarding lung health and comorbidities:

3 % of patients were diagnosed with pulmonary fibrosis.
27 % presented with pulmonary emphysema.
9 % showed signs of bronchiectasis.
55 % were found to have coronary artery calcification.

Protocols

The Czech Republic’s initiative to establish formal ILA management protocols ensures that patients at risk of progression receive timely monitoring.

About the “Turn Your Back on Cancer” Campaign

The campaign ran from November 2025 to March 2026 and aimed at raising awareness of the possibilities of population-based cancer screening programmes.

Communication Strategy

A comprehensive communication strategy was implemented to integrate the topic of cancer screening into the public sphere. Initial phase focused on the online environment and social networks was followed by a massive digital campaign designed to both educate the audience and motivate direct action.

A Public Priority

Through a wave of television spots and print media, prevention was positioned as a public priority. To maximize the campaign’s reach, a network of medical professionals and public figures was engaged to transform screening into a nationwide challenge.

The European Union

The campaign was created with the support of the European Union and follows the long-term strategy of the Ministry of Health in the field of prevention and early detection of diseases. “We want health to be taken care of before problems arise, and for prevention to become a regular part of life. If we approach it in time, we can live fully and really show cancer our back,” adds Karel Hejduk, the director of National Screening Center.

Main Screening Programmes

The campaign drew attention to the five main screening programmes in the Czech Republic focused on:

breast cancer
cervical cancer
colorectal cancer
lung cancer
prostate cancer

Participation

Increase in willingness to participate in screening and in participation itself was observed. Physician engagement in offering the programmes has risen.

Reaching Out to 1,200 Long-term Smokers

Strong public response to screening

Participant recruitment for Finland’s lung cancer screening pilot began earlier this year — and the response has been very encouraging

The pilot reached its target of 1 200 long-term smokers from all the five regions in record time, highlighting a clear need for lung cancer screening in Finland.

Combining screening with prevention

Research shows that combining smoking cessation interventions with lung cancer screening is a key element for success. In the pilot, 600 participants will undergo low-dose CT scans at the beginning and end of the study, and all participants are offered help to quit smoking.

The strong interest suggests that people are concerned about their health and willing to take part in screening, while also highlighting a clear need for smoking cessation support.

Aiming for long-term impact

The aim of the pilot is to determine the efficiency of different smoking cessation methods to smoking cessation, and to generate evidence on the feasibility of implementing lung cancer screening within the Finnish healthcare system and to improve the early detection of the disease.

The pilot is conducted as part of Work Package 8 (Facilitation of the New Screening Approach Implementation) in the #EUCanScreen project. Finnish Cancer Registry is leading the pilot in close collaboration with our Hungarian partner NKIP OKPI – National Korányi Institute for Pulmonology

Avoidable Deaths Through the Primary Prevention, Early Detection, and Curative Treatment of Cancer Worldwide

A Population-Based Study

Available Public Health and Clinical Interventions

The International Agency for Research on Cancer (IARC) has published new research estimating the global number of cancer deaths that could potentially be avoided through effective prevention measures, earlier diagnosis, and access to curative treatment. The population-based study examines worldwide cancer mortality and assesses how many deaths among individuals diagnosed with cancer might be prevented using currently available public health and clinical interventions.

Cancer Risk Factors

The analysis shows that approximately 4.5 million deaths among people diagnosed with cancer globally in 2022 could have been avoided. This represents nearly half of all cancer deaths in this population. The largest share of preventable deaths is linked to primary prevention, accounting for about 3.1 million deaths related to modifiable risk factors such as tobacco use, alcohol consumption, excess body weight, infections associated with cancer, and exposure to ultraviolet radiation.

Early Detection and Access to Treatment

In addition to prevention, the study highlights the critical role of early detection and access to treatment. Around 1.4 million deaths could potentially be avoided through improved screening programmes, timely diagnosis, and access to effective therapies. The findings show that cancers such as breast, colorectal, and prostate cancer contribute substantially to deaths that could be prevented through improved health care delivery.

Global Disparities

The research also emphasizes major global disparities. Low and middle income countries experience the highest burden of preventable cancer deaths because prevention programmes, screening coverage, and treatment services remain limited. The findings provide important evidence for governments and health organizations seeking to strengthen national cancer control strategies and expand prevention and early detection programmes.

European Policymakers and Researchers Discuss Expanding Evidence-based Cancer Screening Programmes

European Cancer Screening and Early Detection Policy Summit

Scientists from the International Agency for Research (IARC) on Cancer participated in the European Cancer Screening and Early Detection Policy Summit. It held on 17 and 18 February 2026 in Brussels and online. The event brought together policymakers, cancer researchers, clinicians and patient organizations. They discussed strategies for expanding screening and early detection programmes across Europe.

Translating Scientific Evidence into Policy Decisions

The summit focused on translating scientific evidence into policy decisions that can strengthen population-based screening programmes. Discussions included the potential expansion of screening initiatives for cancers such as lung, prostate and gastric cancer, as well as the implementation of updated European recommendations on cancer screening.

Key Theme

During the event, IARC researchers presented scientific evidence related to screening effectiveness, programme implementation and quality assurance systems. A key theme was the importance of strong data infrastructure to monitor participation rates, evaluate outcomes and ensure the quality and equity of screening programmes. Participants also examined practical challenges associated with implementing new screening initiatives, including resource allocation, workforce capacity and public engagement.

Organization

The summit was organized by the European Cancer Organisation and supported by the European Health and Digital Executive Agency under the EU4Health programme. The discussions highlighted the importance of collaboration between research institutions, health authorities and civil society organizations in advancing effective screening policies.

Lung Cancer and Screening

Lung cancer is the most common cause of cancer-related death in Ireland – this effect is largely driven by late diagnosis, where 4 in 5 patients are diagnosed with late stage disease. On the contrary, early-stage lung cancer has cure rates of 80-90% with surgery or radiation treatment. Clinical trials from both Europe and the US have demonstrated that lung cancer screening with low-dose CT is associated with a 20% reduction in lung-cancer specific mortality. Despite this evidence, only 2 countries in the EU have national screening programmes.

What is the Lung Health Check?

We are proud to be leading Ireland’s first lung-cancer screening pilot (Principal Investigator: Prof J Naidoo, RCSI/Beaumont; Clinical Lead: Prof D Ryan, Primary care Lead: Prof P Redmond), which aims to save lives by diagnosing lung cancer early – funded through clinician-led research grants from the European Union (EU4Health programme) and the Irish Cancer Society. To lead this effort, we developed Ireland’s first multidisciplinary team for lung cancer screening including GPs, patients, respiratory consultants, surgeons, radiologists and hospital management. With this team, we designed a community-based lung cancer screening pilot using a mobile low-dose CT truck, and invited 22,000 participants with the goal of enrolling 2183 eligible participants over the course of 1 year in the North Dublin/Co. Louth area. Eligibility for the screening pilot is based on meeting high-risk criteria for lung cancer which indicate that the benefits of the low dose CT, outweighs harms. In our study, we are using the Prostate, Lung, Colorectal and Ovarian Risk score and the Liverpool Lung Project Score. The pilot commenced on May 19^th of 2025. The project is supported through a key collaboration with primary care providers (Centric Health), thoracic surgery (Trinity St. James’s Cancer Institute), the National Screening Service (NSS) and Alliance Medical.

What is Involved?

The ‘Lung Health Check’ involves low-dose CT but also a comprehensive assessment of lung health including symptom questionaries, breathing tests (spirometry), smoking advice (if currently smoking) and optional blood/breath/sputum collection for future research. If a suspected lung cancer is identified, participants are seen within 2 weeks at one of our dedicated rapid access lung cancer clinics (Beaumont Hospital or Our Lady of Lourdes Hospital). For those without signs of lung cancer, they have a follow-up imaging in 1 years’ time. In the event that a non-lung cancer finding is identified (for example in the heart or the thyroid gland that can sometimes be seen on lung CT scans), cases are discussed at dedicated lung cancer screening multidisciplinary team meetings at Beaumont Hospital. If required, further referrals are made directly from the study team in Beaumont Hospital to tertiary care. Primary care physicians are updated with CT scan results and any other necessary tests/investigations.

Future Directions

Thus far, the study is progressing with over 1000 participants recruited to date. Early results indicate excellent community engagement with participation rates of 70-75%. We expect to complete enrolment in Q1 of 2026. We expect completion of all screening scans by Q2 2027. We ultimately hope that our work will be a catalyst to the development of a national lung cancer screening programme in Ireland.

Building the Evidence Base for Equitable Lung Cancer Screening

PRiCAN Team

The PRiCAN team at the Royal College of Surgeons in Ireland (RCSI) is addressing a challenge facing health systems across Europe: how do we ensure lung cancer screening programmes reach those who need them most? Through EUCanScreen, we are synthesising international evidence to support stronger implementation across Member States.

What Works in Practice

Our work focuses on a straightforward question: what works to increase participation in lung cancer screening, particularly among underserved populations? Our umbrella review examines 24 studies from eight countries, covering interventions tested in diverse healthcare systems from opportunistic screening in primary care to organised population programmes. The evidence base, whilst growing, reveals consistent patterns.

Patient navigation demonstrates the strongest effect. Studies show that trained navigators, whether community health workers, nurses, or peer supporters, can increase screening uptake by 10-30 percentage points (depending on baseline uptake) (Yang et al., 2025). The mechanism appears straightforward: navigators address practical barriers (transport, appointment scheduling, language), answer questions, and provide continuity through the screening pathway.

Multi-component interventions also show promise, though with greater variability. Combining personalised invitation letters, educational materials tailored to literacy levels, and decision aids improves uptake compared to standard invitations alone. Shared decision-making tools, particularly those incorporating lung cancer risk calculators, support informed choice. While some studies have shown the use of shared decision- making aids to increase lung cancer screening uptake, effectiveness depends heavily on how they’re integrated into consultations.

Contributing to European Knowledge

Our findings feed into EUCanScreen’s work package examining cancer screening implementation barriers across Member States. We are working with partners in Latvia, Finland, the Netherlands, Croatia, and Hungary to understand which recruitment interventions work best in lung cancer screening eligible populations and the sub cohorts within this group.

Next Steps

This work directly supports PRiCAN’s broader mission to strengthen primary care cancer research across Ireland and Europe. Our EUCanScreen findings will inform not just lung cancer screening but broader efforts to reduce inequalities in early detection across cancer types.

Insights from the Conference on Personalised Cancer Screening in Riga, Latvia

On 8 October, the international conference “Personalised Cancer Screening – Lessons Learned and the Way Forward” took place in Riga, at the University of Latvia (LU). Throughout the day, participants discussed topics such as the ethical and social aspects of personalised screening, as well as risk-based screening models for cervical, breast, colorectal, prostate, lung and gastric cancers.

Paolo Giorgi Rossi from the Local Health Authority of Reggio Emilia (Italy) gave a presentation on the conceptual framework of risk-based approaches to cancer screening. “The EUCanScreen Joint Action aims to implement the 2022 European Commission recommendations on cancer screening. Risk stratification is a central element of these recommendations,” noted Rossi.

Risk stratification in cancer screening involves dividing individuals into risk groups (for example, low, medium, and high) based on personal factors such as age, sex, family medical history, lifestyle, and others. In practice, this means that the screening strategy is tailored to each person’s level of risk – for instance, people at higher risk may be invited to screening earlier or more frequently, while those at lower risk may be screened later or less often.

The aim of this approach is to promote personalised healthcare, ensuring that screening is individually tailored. This can improve early disease detection among those most at risk, reduce unnecessary testing for people at low risk, and make more efficient use of healthcare resources.

Conference sessions also highlighted the latest technological developments, including the use of liquid biopsies, polygenic risk scores (PRS), and the role of artificial intelligence (AI) in cancer screening.

Jessie Gomers, from Radboud University Medical Center (the Netherlands), discussed the application of AI in cancer screening. She emphasised that research shows AI accuracy in mammogram analysis is, on average, comparable to that of radiologists. “AI is no longer just a research topic for cancer detection. It is already implemented in some regions in Sweden and Denmark due to a radiologist shortage,” Gomers explained.

In these regions AI is used in cancer diagnostics in two main ways:

Decision support – AI highlights potentially malignant areas, while the final decision on further action remains with the radiologist.
Triage the examination to single or double reading – currently, each screening examination is reviewed by two radiologists, which creates a high workload. By using AI, it is possible to identify high-risk mammograms (which still require double reading) and low-risk ones (which may be reviewed by a single radiologist).

This approach has already shown a significant impact – cancer detection rates have increased, while workload has decreased.

While AI is being integrated into cancer diagnostics, its potential for use within risk-based approaches is also being explored. “I think that in the future AI might be able to predict an individual’s risk of developing cancer within a certain timeframe,” Gomers noted.

The final plenary session addressed the importance of effective communication and citizen engagement in implementing personalised cancer screening programmes across healthcare systems.

Director of the Institute of Clinical and Preventive Medicine at the LU, scientific lead of the Joint Action EUCanScreen, and the conference idea author Marcis Leja notes: “The EUCanScreen Joint Action enabled us to organise this conference dedicated to personalised cancer screening, bringing together leading international experts from both EUCanScreen partner institutions and beyond. Personalised approaches to cancer screening represent our future – and preparation for that future must begin today. The conference provided our specialists not only with the opportunity to follow international developments, but also to establish valuable contacts for future collaborations.”
